Dofetilide dosing schedule

1. Check baseline QTc:
   
   

   >440 ms	Do not use
   <440 ms	Proceed to step 2

   
   
   
2. Calculate CrCl: (in ml/min)
   
   

              (140-age) x body wt(kg)
    CrCl =    ------------------------
               72 x creatinine(mg/dl)
        female: mult by 0.85

3. Dose based on CrCr:
   
   

   ›60	500 mcg bid
   40-60	250 mcg bid
   20-40	125 mcg bid
   ‹20	Do not use

   
   
4. Dose adjustment:

   check ECG 2-3hrs after 1st dose

   Increase in QTc	Action
   ≤15%	continue current dose
   ›15% OR QTc›500ms	Decrease dose



5. If QTc >500ms any time after 2nd dose: discontinue dofetilide
